Mission:
The Circle of Fitness is a clinical, research based exercise program established to provide a gym-based exercise to the patients of the Hebrew Rehabilitation Center (HRC.) Research on aging has shown a clear link between the decrease in muscular strength and muscle mass and increased physical frailty and falls, functional decline, and impaired mobility. The primary goal of Circle of Fitness (CoF) is to preserve the functional independence of our patient participants through a program of progressive resistance training, complimented by cardiovascular exercise. Balance and flexibility exercise may also be included in the exercise plan. In addition to the physiologic benefits of exercise, our program has the potential to enhance the social and psychological well -being of our patients. Progressive Resistance Training (PRT) exercise with a frail elderly population was developed and studied by researchers here at Hebrew Rehabilitation Center''s Research & Training Institute two decades ago.

About Hebrew Rehabilitation Center NewBridge - Hebrew Rehabilitation Center at NewBridge on the Charles in Dedham is located in the Gloria Adelson Field Health Center. HRC NewBridge provides long-term care households, including memory support neighborhoods, and short-term post-acute care on the Rehabilitative Services Unit (RSU).

These internships are available and coordinated through the college or university where the student is enrolled. The student should be in good standing in their program and show interest and enthusiasm for working with older adults. A meeting will be arranged with the head of the particular discipline to help the prospective intern determine if the placement is a good match.

Students must meet all of HSL's external learner onboarding requirements to be offered a rotation.

HRC Roslindale provides long-term care, including memory care, short-term post-acute care on the Rehabilitative Services Unit (RSU), and long-term post-acute care on the Medical Acute Care Unit (MACU).

These internships are available and coordinated through the college or university where the student is enrolled. The student should be in good standing in their program and show interest and enthusiasm for working with older adults. A meeting will be arranged with the head of the particular discipline to help the prospective intern determine if the placement is a good match.

Expressive therapists provide psychosocial support to HSL patients and residents who experience difficulties adjusting to aging, transitioning to a communal living, health status change, memory impairment, depression, anxiety, etc. They use non-verbal modalities of music, movement, and art as vehicles for achieving clinical goals. We offer both group and individual treatment. Members of the ET staff are highly skilled in their primary modality and are seasoned counselors. We have an impressive record of our former trainees who are successfully employed in the expressive therapy and mental health counseling fields. We provide students with weekly supervision by certified expressive therapist and approved LMHC supervisor as appropriate. The CCB campus is accessible by MBTA.

The mission of the Pharmacy Department is to provide services that contribute to optimal pharmaceutical care for all patients served by Hebrew Rehabilitation Center (HRC). Pharmacists will provide patient-centered and population-based care that optimizes medication therapy to manage health care system resources, to improve therapeutic outcomes and to promote health improvement, wellness and disease prevention.

The Pharmacy's goal is to participate as a member of the health care team to ensure safe and effective medication utilization and optimal patient outcomes. Our responsibilities include medication preparation, distribution, medication monitoring, provision of drug information and training of pharmacy students to be able to meet the needs of the geriatric population. Our intent is to support research that explores opportunities for the betterment of human life.

The staff believe that a core part of their mission is to be educators to:
-Provide educational services as needed to nurses, physicians, and other clinical staff members.
-Support programs that facilitate the pharmacotherapeutic management of disease states and drug utilization.
-Facilitate programs that will help future pharmacists in meeting the needs of the geriatric population.
-Act as a leader in providing information for pharmacotherapeutic management of disease states.

Advanced Pharmacy Practice Experiences

The purpose of Advanced Pharmacy Practice Experiences (APPE) is to apply the facts, information, and concepts gained during classroom and didactic learning and apply it to real-life experiences and learning. The APPE experience at HSL allows students to use an integrative approach to learning that includes experiences and interactions between the students and HSL pharmacy team members to turn their classroom knowledge into practical knowledge.

Hebrew Rehabilitation Center in Roslindale is a Long Term Care Hospital which provide services to Recuperative Service Unit (RSU), Medical Acute Care Unit (MACU), Medical Care Long Term Care Hospital (MCLTCH) and Long Term Chronic Hospital (LTCH).

Students are involved in the following activities:
1. Perform medication reconciliation on patients who are admitted to the MACU, MCLTCH and RSU.
2. Learn daily routine of Pharmacy operations: Omnicell (includes carousel), unit dose packaging of medication, observe IV medications preparation. Shadow daily work of pharmacist
3. Monitor patients who are anticoagulation therapy
4. Evaluation the appropriateness of vancomycin IV dosing and trough
5. Review Adverse Drug Reaction (ADR) report and report to Pharmacy and Therapeutics Committee (P&T) if deemed appropriate
6. Attend P&T and present topics for medical staff as needed
7. Access patients' medication list who have high fall risk and suggest alternative treatment/plan to providers.

Introductory Pharmacy Practice Experiences

The purpose of Introductory Pharmacy Practice Experiences (IPPE) at HSL is to help students develop the foundational knowledge, skills and attitudes for future long-term care and hospital pharmacy practice while instilling professionalism on IPPE students. The IPPE course will expose students to the role of the long-term care pharmacist and pharmacy practice setting.

Responsibilities:

Hebrew Rehabilitation Center in Roslindale is a Long Term Care Hospital which provide services to Recuperative Service Unit (RSU), Medical Acute Care Unit (MACU), Medical Care Long Term Care Hospital (MCLTCH) and Long Term Chronic Hospital (LTCH).

By the end of the IPPE rotation, students will have gained the following competencies:
1. Understand and articulate the pharmacist''s role in medication safety and quality improvement activities
2. Educate patients, families and caregivers about the importance of health, wellness and disease prevention and provide effective health and medication counseling as needed/requested
3. Accurately prepare, label, dispense and distribute medications
4. Demonstrate the ability to work as a member of the interprofessional team
5. Demonstrate appropriate use of drug information resources to assist in patient care
6. Describe the steps required to ensure facility compliance with accreditation, and legal, regulatory and safety requirements.

The Social Services Department has licensed social workers who are responsible for providing support, advocacy and concrete services to patients, residents, and their families.

Center Communities of Brookline offers independent, affordable, non-denominational apartment-style living with on-site services that promote independence and wellness to seniors and young adults with disabilities. A staff of 30 provides care and service on the campus. They have an interdisciplinary team, including social work, fitness, programming and spiritual care.

Interns have the opportunity to work with older adults both in group settings and individually, either in an office or in the client's apartment. They also experience the benefits of being on an interdisciplinary team and have opportunities to attend team meetings and collaborate with other disciplines to provide support to their clients. There is often an opportunity to work on a project that may interest them over the course of the year.

Students participating in internship experiences must be coming from schools with an existing affiliation agreement and must meet all of Hebrew SeniorLife's eligibility requirements for external learners.
