Description

The mission of the Pharmacy Department is to provide services that contribute to optimal pharmaceutical care for all patients served by Hebrew Rehabilitation Center (HRC). Pharmacists will provide patient-centered and population-based care that optimizes medication therapy to manage health care system resources, to improve therapeutic outcomes and to promote health improvement, wellness and disease prevention.

The Pharmacy's goal is to participate as a member of the health care team to ensure safe and effective medication utilization and optimal patient outcomes. Our responsibilities include medication preparation, distribution, medication monitoring, provision of drug information and training of pharmacy students to be able to meet the needs of the geriatric population. Our intent is to support research that explores opportunities for the betterment of human life.

The staff believe that a core part of their mission is to be educators to:
-Provide educational services as needed to nurses, physicians, and other clinical staff members.
-Support programs that facilitate the pharmacotherapeutic management of disease states and drug utilization.
-Facilitate programs that will help future pharmacists in meeting the needs of the geriatric population.
-Act as a leader in providing information for pharmacotherapeutic management of disease states.

Advanced Pharmacy Practice Experiences

The purpose of Advanced Pharmacy Practice Experiences (APPE) is to apply the facts, information, and concepts gained during classroom and didactic learning and apply it to real-life experiences and learning. The APPE experience at HSL allows students to use an integrative approach to learning that includes experiences and interactions between the students and HSL pharmacy team members to turn their classroom knowledge into practical knowledge.

Hebrew Rehabilitation Center in Roslindale is a Long Term Care Hospital which provide services to Recuperative Service Unit (RSU), Medical Acute Care Unit (MACU), Medical Care Long Term Care Hospital (MCLTCH) and Long Term Chronic Hospital (LTCH).

Students are involved in the following activities:
1. Perform medication reconciliation on patients who are admitted to the MACU, MCLTCH and RSU.
2. Learn daily routine of Pharmacy operations: Omnicell (includes carousel), unit dose packaging of medication, observe IV medications preparation. Shadow daily work of pharmacist
3. Monitor patients who are anticoagulation therapy
4. Evaluation the appropriateness of vancomycin IV dosing and trough
5. Review Adverse Drug Reaction (ADR) report and report to Pharmacy and Therapeutics Committee (P&T) if deemed appropriate
6. Attend P&T and present topics for medical staff as needed
7. Access patients' medication list who have high fall risk and suggest alternative treatment/plan to providers.
