Description

The mission of the Pharmacy Department is to provide services that contribute to optimal pharmaceutical care for all patients served by Hebrew Rehabilitation Center (HRC). Pharmacists will provide patient-centered and population-based care that optimizes medication therapy to manage health care system resources, to improve therapeutic outcomes and to promote health improvement, wellness and disease prevention.

The Pharmacy's goal is to participate as a member of the health care team to ensure safe and effective medication utilization and optimal patient outcomes. Our responsibilities include medication preparation, distribution, medication monitoring, provision of drug information and training of pharmacy students to be able to meet the needs of the geriatric population. Our intent is to support research that explores opportunities for the betterment of human life.

The staff believe that a core part of their mission is to be educators to:
-Provide educational services as needed to nurses, physicians, and other clinical staff members.
-Support programs that facilitate the pharmacotherapeutic management of disease states and drug utilization.
-Facilitate programs that will help future pharmacists in meeting the needs of the geriatric population.
-Act as a leader in providing information for pharmacotherapeutic management of disease states.

Introductory Pharmacy Practice Experiences

The purpose of Introductory Pharmacy Practice Experiences (IPPE) at HSL is to help students develop the foundational knowledge, skills and attitudes for future long-term care and hospital pharmacy practice while instilling professionalism on IPPE students. The IPPE course will expose students to the role of the long-term care pharmacist and pharmacy practice setting.

Responsibilities:

Hebrew Rehabilitation Center in Roslindale is a Long Term Care Hospital which provide services to Recuperative Service Unit (RSU), Medical Acute Care Unit (MACU), Medical Care Long Term Care Hospital (MCLTCH) and Long Term Chronic Hospital (LTCH).

By the end of the IPPE rotation, students will have gained the following competencies:
1. Understand and articulate the pharmacist''s role in medication safety and quality improvement activities
2. Educate patients, families and caregivers about the importance of health, wellness and disease prevention and provide effective health and medication counseling as needed/requested
3. Accurately prepare, label, dispense and distribute medications
4. Demonstrate the ability to work as a member of the interprofessional team
5. Demonstrate appropriate use of drug information resources to assist in patient care
6. Describe the steps required to ensure facility compliance with accreditation, and legal, regulatory and safety requirements.
